NOTE: By submitting
this form, I acknowledge and agree to the following
conditions of LAHPA participation: 1. I am at least
eighteen (18) years of age. 2. I hereby absolve and
release the LAHPA, its officers, players, ice rinks
(including Iceland Sports Complex), rink staff, and
management, and all others involved in any way with
this league from liability for any injury to person
or property incurred by or as a result of my
participation in this league. 3. I acknowledge that
I play completely at my own risk!!! 4. I understand
that this is a full slapshot, NO CHECK league with
the following major rules of play: a. No checking /
hitting on open ice or against the boards, no
carrying someone into the boards. b. Contact is to
be avoided, recognizing that "incidental contact"
in unavoidable. c. Avoiding injuries is foremost in
everyone's mind. Everyone has fun and returns to
work on Monday morning. d. Players may "rub another
off the puck" if their position allows. e. Players
along the boards may "contain" each other, pinching
them off. f. Play in front of the net must exclude
slashing, crosschecking, hooking, etc..... g. USA
Hockey rules and regulations will be followed. h.
Facemasks are mandatory (half-shield minimum) 5. I
agree that fighting will not be tolerated, and will
result in expulsion from the game (game
misconduct), and expulsion from the league. 6. I
agree at all times to abide by USA Hockey’s Codes
of Conduct for Players listed below • Promote
safety and sportsmanship. • Play for fun. • Work
hard to improve your skills. • Be a team player -
get along with your teammates. • Learn teamwork,
sportsmanship and discipline. • Be on time for
practices and games. • Learn the rules and play by
them. Always be a good sport. • Respect your coach,
your teammates, your parents, opponents, and
officials. • Never argue with an official’s
decision.
